Use GreenLife to be verified online and register as a member of our Collective. Once you are a member, you can visit our club.

After filling out the forms above, be sure to email us a copy of your drivers license and your full page 8"x11" doctors recommendation to.... sales@rxpotshop.com to be pre-verified, or bring in a copy with you when you come in.

We can not pre-verify you without these documents!

Note: Internet Pre-Verification may take upto 12 hours. Please call ahead or come in to our club during your doctors regular business hours if you need imediate help. Members who are not pre-verified may have longer wait times while we try to verify you. Original documents (or doctors who use internet verification or automated 24/7 call centers) may expedite your verification process. Some doctors are not available for telephone verifications after hours, weekends, and/or holidays.
