After filling out the forms above, be sure to email us a copy of your drivers license and your full page 8"x11" doctors recommendation to.... sales@rxpotshop.com to be pre-verified. Also please bring in a copy with you when you come in.

We can not pre-verify you without these documents!

Note: Internet Pre-Verification may take upto 2 hours. Please call ahead or come in to our club during your doctors regular business hours if you need imediate help. Members who are not pre-verified may have longer wait times while we try to verify you. Original documents (or doctors who use internet verification or automated 24/7 call centers) may expedite your verification process. Some doctors are not available for telephone verifications after hours, weekends, and/or holidays.
